The Role:
We're seeking a compassionate and enthusiastic Support Worker to join our growing WA team. In this role, you'll provide tailored support to our client who has a neuromuscular condition. You'll be assisting her with daily activities, community engagement, and achieving her personal goals.
Our client lives independently in the Fremantle area with her cats. She is creative young woman who values consistency, a sense of humour and her independence. She benefits from support workers who are energetic, confident and open minded. This role involves use of a hoist and manual handling.
Days & times to be discussed with the successful person, however morning shifts start at 7am and end anywhere between 10am and 1pm. Evening shifts usually start at 5pm and end at roughly 7pm. The current vacancy is for ongoing Monday morning shifts and frequent cover shifts - most members of our team travel frequently, so there are lots of opportunities for extra work
Support Required: Assistance with personal care, daily living and community access. Taking the client swimming, meal preparation, cleaning.
Bonus: Experience driving a wheelchair accessible van is highly regarded.
Key Responsibilities:
* Assist client with daily living tasks, including personal care and household duties
* Support client in community participation and social activities, including going swimming regularly
* Collaborate with client to develop and implement individualized support plan
* Maintain accurate and timely documentation of services provided
Requirements:
To be considered for this role, you must have or be willing to obtain:
* National Police Clearance
* CPR and First Aid Certificate
* Manual Handling Training
* NDIS Workers Orientation Module Certificate
* NDIS Worker Screening Clearance
* Working with Children's Clearance
* 100 points of ID
* COVID-19 Vaccination Certificate
