You'll move through a mix of complex clinical assessments, thoughtful case planning and hands-on collaboration with clients, families and the multidisciplinary team.
Your work will centre on coordinating tailored rehabilitation pathways, guiding team members, and navigating clinical challenges with confidence and empathy.
You'll contribute to quality improvement, mentor colleagues, and support a positive practice environment where best-practice care thrives.
Every interaction—whether problem-solving with allied health, reviewing eligibility assessments, or advocating for client needs—will allow you to use your advanced clinical skills to create meaningful, measurable impacts in the Transition Care Program.
