The ideal candidate will have:Recent experience as a Transition Care Program (TCP) coordinator or in a complex care coordination roleExcellent communication skills, both written and verbalAbility to undertake comprehensive assessments to:Determine client eligibility for our programAssist in developing client-centered, goal-oriented care plansAbility to liaise with a broad range of stakeholders along with leading the multidisciplinary teamStrong organisational skillsUnderstanding of the new Aged Care Act and Aged Care Quality StandardsIn this role you will coordinate and facilitate the transition of older people from hospital back into the community through assessment, care planning, referral and support.
