About This Role
The Transition to Residential Aged Care Unit is a pioneering 20-bed ward where acute care nursing intersects with residential aged care. Patients have recovered from an acute medical episode but are no longer safe for discharge home.
Here, we collaborate closely with families and carers to support patients as they transition into an Aged Care facility.
As a TRAC nurse, you'll provide comprehensive geriatric clinical care, palliative support, and essential social and emotional care to patients and their loved ones.
