Our North Queensland office, under new management, seeks experienced and dedicated Care Coordinators to support regional growth.
Role Overview
The role involves service development, coordination, client assessments, care plan development, and direct nursing support. A current AHPRA registration (EN or RN) is required. The successful candidate will enjoy working in a team and delivering high-quality client care.
Responsibilities include:
- Care assessments
- Care planning compliant with government standards
- Nursing supports
- Building client relationships
- Quality assurance

Qualifications
- Diploma in Nursing with current AHPRA registration
- Valid driver's license and insured private vehicle
- Current Police Clearance
- First Aid & CPR certification
Preferred Skills
- Understanding of aged care funding (CDC) & NDIS (preferred)
- Knowledge of hospital discharge processes
- Team-oriented, caring, and quality-focused attitude
- Excellent communication, professionalism, and respect
- Good judgment, decision-making, staff management, and administrative skills
