**About the role**:
We are seeking enthusiastic, passionate, and caring individuals, with good communication skills to join our team. Your work will support people to achieve their individual lifestyle goals. You will work with participants in the Community, providing a one on one service. As a support worker, you will support the independent living of clients through personal care, community participation, and assistance with daily personal activities.
**Responsibilities and Duties**:
Provide one on one direct care assistance to each participant in accordance with their care plan.
Monitor the safety and well-being of the participant and report any concerns to the supervisor.
Support clients to access activities and community.
Manage accurate and detailed progress reports and incident reports.
Work with the participant, family, and other professionals to support the participant's individual goals.
**To be considered for this role, you MUST provide the following**:
NDIS Worker Screening Clearance
Working With Children's Check (Employee)
Current First Aid & CPR Certificate
Current Driver's License & your own car
A Certificate III or IV in one of the following or equivalent: Individual Support/ Home & Community Care/ Aged Care/ Disability/ Nursing
Covid 19 Vaccinations
Police CHeck
