Role Summary
This is a skilled social work position requiring the provision of high-quality services to clients in need.
* Key Responsibilities:
o Conduct comprehensive psychosocial assessments and risk evaluations to inform client care planning and discharge strategies.
o Offer expert advocacy and support for patients, carers, and families, ensuring their voices are heard and needs met.
o Prioritize crisis intervention counseling to help individuals cope with disease, disability, loss, grief, and bereavement.
o Collaborate with multidisciplinary teams to address complex cases of child abuse, elder abuse, and domestic violence.
