About This Role
As Nurse Navigator – Movement Disorders, you will provide advanced clinical and professional leadership to facilitate seamless, patient-centred care across the continuum.
You'll collaborate with multidisciplinary teams to improve outcomes for patients with complex neurological conditions and support system improvements in care delivery.
You Will
Coordinate and deliver specialised nursing care for patients with movement disorders
Provide advanced clinical assessment, education and care planning across service settings
Collaborate with multidisciplinary teams and external providers to optimise care pathways
Lead quality improvement, service innovation and workforce development initiatives
About You
You are a skilled senior nurse who excels in care coordination, critical thinking and collaboration.
You demonstrate advanced clinical expertise in neurological or chronic disease nursing and are passionate about improving patient experiences and outcomes.
