About this role:
The Clinical Care Coordinator will provide high-quality evidence-based clinical care as a member of the multidisciplinary team delivering care coordination services for children and youth, focusing on early intervention and support of young people up to the age of 25.
The role involves addressing service gaps for the 'missing middle' by enhancing accessibility and integrating primary mental health care, psychosocial support, and early intervention.
Promote holistic wellbeing by considering broader determinants of health, fostering collaboration among providers, and ensuring a sustainable, adaptable service responsive to community needs.
* Based in Rockhampton with flexibility to work from home one day per week

Key qualifications:
* Must have relevant registration/eligibility as an Occupational Therapist, Mental Health Nurse, Psychologist/Clinical Psychologist, Social Worker or Allied Health Professional
* Knowledge and understanding of the theory underpinning evidence-based interventions and clinical practice in mental health and alcohol and drug use
* Highly developed communication and interpersonal skills
* Ability to engage with people, advocate on their behalf and establish and maintain effective relationships
* Experience in electronic client/customer management systems and Microsoft Office applications
* Must be eligible to apply/currently hold a Blue Card and NDIS worker screening clearance
