About the Opportunity
As the Community Care Nurse Unit Manager, you will be responsible for the strategic and operational delivery of community nursing services for the Western Cluster of MLHD. In managing your community nursing team, you will take a proactive role in overseeing and driving continuous improvement initiatives to strengthen service delivery and achieve improved health outcomes for the community.
As the Community Care Nurse Unit Manager, you will:
* Provide clinical leadership to the community nursing team to ensure the exceptional delivery of evidence-based care within contemporary models of practice. Ensure governance processes are in place, including timely and appropriate responses to incidents, complaints, and risk management.
* Lead and manage processes that drive quality outcomes, ensure patient safety, and support continuous improvement—demonstrated through the achievement of key performance indicators, outcome measures, patient and staff surveys, and audit results
* Work collaboratively with the Rapid Access clinic (RAC) and Hospital in the Home (HITH) leadership teams to ensure service provision is client centred and effectively integrated with other services available to the local community to promote health and wellbeing outcomes.
* Manage the efficient use and coordination of physical and financial resources to maximise expenditure toward health outcomes and ensure plans are in place to promote business continuity and avoid potential service disruptions
* Lead with purpose by motivating and managing teams, applying change management to address challenges in community and rural health service delivery.

About You
Our ideal candidate will demonstrate:
* Current authority to practice unrestricted as a Registered Nurse with the Australian Health Practitioner Regulation Agency (AHPRA).
* Class C drivers licence
* Proven experience in a leadership or management role within a healthcare setting.
* Excellent communication and interpersonal skills.
* Commitment to continuous improvement and professional development.
* Demonstrated understanding of Community Nursing.
