About the Role
The Clinical Lead is the senior clinician responsible for delivering care management within Trilogy’s End-of-Life Pathway and providing clinical oversight for the Restorative Care Pathway under the Support at Home Program.
Reporting to the Head of Care, this office-based role involves providing hands-on care management to clients in the End-of-Life Pathway while overseeing the clinical quality and supervision of Restorative Care Partners. Working closely with internal teams, the Clinical Lead ensures both models are clinically sound, compliant, and responsive to the needs of clients and the organisation.
Key Responsibilities
End-of-Life Care Management
* Deliver care management for clients on the End-of-Life Pathway
* Conduct assessments, lead multidisciplinary coordination, and develop care plans
* Partner with clients, families, and providers to ensure care is timely, compassionate, and person-centred
* Support advance care planning and family/carer education
* Provide ongoing clinical monitoring and pain/symptom assessment
* Support cultural safety, dignity, and informed choice in all interactions
Clinical Oversight and Operational Leadership
* Provide clinical leadership across the End-of-Life and Restorative Care Pathways
* Supervise and support Restorative Care Partners with clinical guidance and performance oversight
* Maintain direct oversight of clinical quality, escalation processes, and model fidelity
* Support compliance with care management requirements
* Provide input into caseload allocation and care management monitoring
* Facilitate and participate in case conferencing as required
* Facilitate tailored training to end-of-life and restorative care partners
Pathway Support and Improvement
* Ensure alignment of pathway delivery with the Aged Care Act 2024, Support at Home Program Manual, and Quality Standards
* Contribute to iterative improvements based on practice insights, quality data, and stakeholder feedback
Stakeholder Engagement
* Coordinate with and build effective relationships with state/territory palliative care services and healthcare providers
* Collaborate with internal and external teams to ensure coordinated delivery across care and quality functions
Measures of Success
* Effective delivery of care management to End-of-Life clients
* Positive outcomes in client experience, wellbeing, and care goal achievement
* High clinical performance and engagement from Restorative Care Partners
* Pathway compliance with quality and program standards
* Strong internal and external stakeholder relationships
Qualifications and Experience
Required:
* Registered Nurse with current AHPRA registration
* Demonstrated experience in palliative care, aged care, or community health settings
* Proven capability in leading and supervising clinical care and multidisciplinary teams
Desirable:
* Knowledge of aged care reforms, including the Support at Home Program
* Strong communication and stakeholder engagement skills
* Ability to lead with empathy, accountability, and a commitment to continuous improvement
