About the Role
Our experienced Support Coordinators collaborate with participants and their families to understand their NDIS plans, identify suitable service providers, and develop essential skills in self-management.
* Support participants in achieving their goals and provide expert coordination services.
* BUILD strong networks and collaborate with stakeholders to identify and implement effective supports.
* Prepare accurate case management notes and reports in our secure management database.
Key responsibilities include ensuring compliance with NDIS practice standards, attending professional networking events, and meeting billable hours targets.
Required Skills and Qualifications
The successful candidate will possess a valid driver's license, undergo a criminal history check, NDIS workers screening check, and working with children's check.
