Aged Care Support Connector

WHAT WE DO:

Galangoor works with the local Indigenous community and relevant key stakeholders to increase access to culturally safe, quality, coordinated and comprehensive primary health care services. We focus on healthcare across the life span, including the management and reduction of chronic disease, to address the health and social inequities of Aboriginal and Torres Strait Islander communities of the Fraser Coast Region.

THE POSITION:

The Aged Care Support Connector plays a vital role in supporting older Aboriginal and Torres Strait Islander people and their families to access culturally safe aged care services. This position focuses on community engagement, advocacy, and navigation of My Aged Care processes, ensuring clients receive the right level of care and support.

You will work closely with the Aged Care Support Coordinator and health teams to identify service gaps, promote aged care entitlements, and connect clients with internal and external providers.

KEY RESPONSIBILITIES:

Active community outreach to promote aged care services, identify and engage with potential clients.

* support clients to identify goals and needs to ensure they get the appropriate level of care and services, which can include referrals to other specialised services and support.
* support clients' aged care needs through internal referral processes.
* assist clients with advocacy and support throughout the My Aged Care registration and review processes.
* Provides support to enable client access to appointments.
* Liaises with the aged care team to continuously review and enhance the role.
* Identify available services and access pathways, and referrals to other specialised services and support.
* Provide advocacy through the assessment process.
* Support clients to identify and select aged care service providers.
* Support clients to understand contractual arrangements and costs of care.
* Provide cultural support during the development of a client's care plan, and throughout the access journey.
* Link in with existing aged care advocacy services, as required.
* Deliver face-to-face engagements to promote their role and aged care supports available and
* Develop and disseminate local, place-based communications materials and resources.

KEY REQUIREMENTS:

Essential:

No qualification is required for this role.

Desirable

Qualification in Community Services and Aboriginal Mental Health First Aid

Job Specific Capabilities

* Excellent and strong relationships with the local Aboriginal and Torres Strait Islander Community
* Good communication and interpersonal skills, with the ability to build trusting relationships in community.
* Demonstrated understanding of the principles of person-centred care and trauma-informed practice.
* Experience working with older people.
* Understanding of the internal ACCHO services available to clients.
* Knowledge of or willingness to learn about the aged care landscape.
* Ability to or willingness to learn how to enter data and reporting.
* Demonstrated experience working unsupervised and as part of a team.

Mandatory Qualifications/Professional Registrations and Licences:

* Current C Class Drivers Licence (QLD)
* Current Blue Card
* Current annual CPR Certificate
* Current First Aid Certificate (every three years)
* Criminal history checks will be undertaken on the preferred applicant(s). (A criminal conviction or charge will not automatically exclude an applicant from consideration for appointment. * Recommended Immunisations:

* In accordance with Practice Team Immunisation Policy

WHAT WE ARE OFFERING:

Salary Package

* Competitive base salary + super + salary sacrifice + annual leave
* Clinic Days are Monday – Friday
* Rostered 7.6 hours between the hours of 8 am to 6 pm.
* Occasional attendance at community events may be out of hours or on the weekend
