Job Description
This team is the single entry point for new referrals into the Area Mental Health Service.
The role is dynamic, fast-paced, and requires a clinician with very strong phone-based triage experience to ensure safe and effective care pathways.
About the role:
Provide recovery-oriented and trauma-informed mental health triage
Conduct screening assessments across the lifespan
Assess and manage risk, allocating follow-up care in line with Victorian Mental Health Triage Response categories
Direct referrals to the most appropriate team or external provider
Support after-hours bed management for acute admissions
Maintain accurate documentation in MasterCare and TRAK
About you:
Registered Psychiatric Nurse (RPN3) with proven experience in mental health triage, clinical decision-making, and crisis assessment
Strong communication and documentation skills
Must have own car and ability to travel (fuel reimbursed within VIC)
Contract details:
Rate:$80.23 - $86.89 per hour + penalties + super
