About the Aged Care Assessment Officer (Home Support) role
Responsibilities
* Conduct non‐clinical Home Support assessments using the Integrated Assessment Tool via My Aged Care.
* Develop Support Plans to help clients achieve goals and maintain independence.
* Ensure compliance with standards, maintain data integrity, and meet regulatory requirements.
* Maintain accurate client data on My Aged Care and meet KPIs for funding.
Qualifications
* Qualification in aged care, health, or a related field (preferred).
* Current, valid Victorian driver's licence.
