About the Role
An opportunity is available for a Response Case Manager within the Carer Support Service (CSS), based at Mt Helen. CSS supports carers across the Central Highlands, Central Grampians and Wimmera regions. The role focuses on person‐centred assessment, care planning and coordination of supports for carers, working flexibly within a supportive multidisciplinary team.
Key Responsibilities
* Assess carer needs and develop support plans
* Coordinate services, referrals and advocacy
* Monitor outcomes and maintain client records
* Work collaboratively with internal and external partners
Skills & Experience
* Relevant qualification in community or health services
* Experience in assessment and care coordination
* Knowledge of carers and community‐based services
* Strong communication and time management skills
* Current driver's licence
