This role is focused on delivering advanced home health care services to clients in the community, emphasizing high-quality patient outcomes and experience.
Key Responsibilities
* Clinical assessment and tailored care planning to meet individual client needs.
* Nursing services encompassing medication management, wound care, diabetes management, catheter care, stoma therapy, and cognitive assessments to promote optimal client well-being.
* Collaboration with healthcare professionals for a multidisciplinary approach to comprehensive care.
* Collaboration with case managers and support coordinators to achieve positive outcomes and enhance client satisfaction.
* Prioritization of tasks and completion of required documentation to a high standard ensuring efficient service delivery.
About You / Qualifications
* AHPRA registration as a Registered Nurse with current unrestricted status.
* Demonstrated expertise in clinical nursing practice.
* Experience working with clients presenting complex and chronic health conditions.
* Excellent verbal and written communication skills.
* Strong time-management and attention to detail.
