The Role
As a Complex Case Manager (Clinical), you will support clients with complex, high-risk or multifaceted needs. Operating within a trauma-informed framework, you will undertake assessments, coordinate multidisciplinary supports, and act as a primary point of contact for clients navigating health, mental health and psychosocial services.
This role suits an experienced clinician who is confident in managing complex presentations, applying strong judgement, maintaining high-quality documentation and partnering with internal and external stakeholders to drive positive outcomes.

Key Responsibilities
* Provide clinical, trauma-informed case management to clients with complex and high-risk needs.
* Undertake intake, triage and risk assessments.
* Manage a caseload of clients requiring coordinated and intensive support.
* Act as the key point of contact for clients, guiding them through processes and connecting them with relevant services and supports.
* Collaborate with internal teams, community organisations, service providers, and other stakeholders.
* Provide specialist advice, coaching and support to colleagues in relation to challenging presentations or risk escalations.
* Escalate high-risk issues appropriately through senior clinical or management channels.
* Prepare high-quality client summaries, documentation and briefs.
* Represent the organisation in internal and external meetings or presentations as required.

Mandatory Requirements
* Bachelor's degree or higher in a relevant clinical or health discipline such as Nursing, Psychology, Occupational Therapy or Social Work.
* Professional experience supporting clients with complex needs.
* Strong understanding of disability impacts and capacity-building models.
* High-level written and verbal communication, with the ability to build effective relationships.
* Proficiency in IT systems, including Microsoft Office.
* Experience in telephone-based triage, assessment and case management.
* Commitment to professional development.
* Current (or ability to obtain) National Police Check.
* Australian citizenship.

* Additional qualifications in disability, mental health, allied health, social work, education or human services.
* Experience supporting individuals with military, trauma or complex mental health backgrounds.
* Knowledge of evidence-based therapeutic interventions for clients with complex support needs.
