The Transitional Aged Care Program (TACP)
TACP provides short-term interventions designed to optimize the functioning and independence of older adults after a hospital stay.
* Discipline-appropriate assessment, discharge planning, and slow-stream therapeutic intervention
* Case management for aged clients in their homes
* Referral to and consultation with other relevant healthcare professionals, service providers, and community support services
We seek a registered nurse who can provide evidence-based nursing treatments with a focus on improved health status, promotion of client independence, and adherence to hospital policies and procedures.
About You
Key requirements include:
* Current registration as a Registered Nurse (Division 1) with the Australian Health Professionals Regulation Agency
* Valid Class C Driver's License or above
* Effective communication and interpersonal skills within an interdisciplinary team environment
* Ability to prioritize competing workloads in a complex environment
* Evidence of integration of theoretical knowledge and clinical skills, critical thinking, and analysis in daily practice
