Blue Mountains District ANZAC Memorial Hospital is an acute hospital that supports a population of approximately 80,000 and is the only ANZAC Memorial hospital remaining in NSW. Easily accessible to Sydney residents, our team of clinicians, allied health, and support staff works with the community on a shared goal to maintain the delivery of high-quality healthcare in the Blue Mountains region. Located in South Bowenfels, the Lithgow Campus is considered as a comprehensive community health centre supporting approximately 20,000 people living in the town and surrounding areas. The services available in Lithgow hospital include Domiciliary care unit, Emergency department, Geriatric assessment unit, Obstetric services, Paediatric service, Drug and Alcohol unit.
An opportunity has become available for an experienced health professional to join our Allied Health team as a Stroke Coordinator. This position reports jointly to the Directors of Nursing and General Managers at both hospital sites. The Stroke Coordinator is a key clinical leadership role responsible for coordinating and enhancing stroke services across Blue Mountains District ANZAC Memorial Hospital and Lithgow Hospital. The position provides expert leadership to ensure the delivery of high-quality, evidence-based stroke care across the patient journey, from prevention and acute management through to rehabilitation and community transition.
Working collaboratively with multidisciplinary teams, the Stroke Coordinator supports compliance with NSW Telestroke Service requirements and national stroke care standards while driving service improvement and optimal patient outcomes. The role plays a key part in strengthening clinical governance, workforce capability, data monitoring and reporting, and quality improvement initiatives to reduce complications and improve patient experience and outcomes. Through strong clinical leadership and partnership with key stakeholders, the Stroke Coordinator supports sustainable service delivery and contributes to the progression toward a Stroke Capable Regional General Hospital model.
What you will bring to the role
* a) Registered Nurse with AHPRA with minimum 5 years full time equivalent post registration experience, with at least 3 years full time equivalent experience in the specialty field and post graduate Nursing qualifications relevant to the field or such other qualifications or experience deemed appropriate by the employer or; b) Degree in Occupational Therapy or recognised qualification by the board and current registration with AHPRA as an Occupational Therapist or; c) Dietitian with tertiary qualification in nutrition and dietetics and eligible for membership with Dietitians Australia or; d) Tertiary qualifications in Speech Pathology and eligible for membership with Speech Pathology Australia or; e) Degree in Social Work from an accredited course providing eligibility for membership of the Australian Association of Social Workers or; f) Tertiary qualification in Physiotherapy and current full national Physiotherapy registration with AHPRA.
* Good understanding of stroke care, its management and outcomes.
* Demonstrated ability to initiate, complete and evaluate quality improvement projects, contributing to the National Standards accreditation tool.
* Demonstrated high level of communication and interpersonal skills, particularly negotiation, with all disciplines.
* Demonstrated experience in managing and facilitating a successful team environment.
* Demonstrated understanding of work health and safety legislation along with a demonstrated ability to develop, manage and maintain databases and analyse database information.
* Level of Information technology literacy in Office automation programs (including word processing, email and spreadsheets).
* Current driver's license and willingness to use for work purpose travel.
