About You
This is a part-time (3 days a week) Identified role for Aboriginal and Torres Strait Islander people and is a contract position until 30 June 2026.
You are an Aboriginal and/or Torres Strait Islander who enjoys working with at-risk young people.
You are:
* experienced in delivering high quality psycho-social assessment and evidence-based, focused psychological interventions
* thrive in team environments and value the benefits of working closely with other team and external service providers to maximise care outcomes for young people
* have the capability and a commitment to actively fostering a culture of youth friendly, socially and culturally inclusive service provision for young people
* known for modelling culturally safe practices within the workplace by demonstrating inclusive behaviours and showing respect for consumers, carers and colleagues from diverse backgrounds, experiences and perspectives.
Key areas of responsibility
The Youth Care Coordinator works collaboratively with young people and their family/carers to identify their support needs, goals and develop collaborative care plans.
Key responsibilities include:
* Engaging young people and families in the services and activities of headspace Shellharbour
* Providing intake service for headspace Shellharbour including triage, assessment and referral support using the headspace assessment tools, policies and procedures
* Monitoring and managing the service's waitlist while the young person is waiting for allocation to a clinician in accordance with organisational policy and procedures
* Working collaboratively with young people to identify their unique support needs and participate in developing and implementing care plans
* Providing care coordination and practical assistance to young people including accessing the services they need, consulting with internal and external service providers regarding care needs, building safety plans with young people and their support networks, and providing person-centred counselling and psychoeducation
* Participating in care plan and case review meetings, and planned case conferences with GPs and other health professionals involved in the young person's care as required
* Building relationships with external service providers and reporting on opportunities for mutually beneficial partnerships that improve outcomes for Aboriginal and Torres Strait Islander young people accessing headspace, and the community
* Record keeping and reporting in line with the organisation's policies and procedures, and national evaluations and minimum data sets
* Participating in cultural supervision.
Essential Criteria
* Aboriginality and an active member with the local Aboriginal communities– your employer considers Aboriginality a genuine occupational qualification for the role in accordance with the Anti-Discrimination Act (s.14(d))
* Qualifications in a relevant field or working towards the same in youth work, community welfare, social work, psychology, peer work, occupational therapy, counselling or other related qualifications or demonstrated experience in a similar role
* Demonstrated understanding of issues affecting the health and wellbeing needs of young people with mental health and/or substance use problems, as well as young Aboriginal people and their families
* Experience in delivering individual and group interventions
* Experience providing coordination of care and ability to work from a recovery-oriented framework
* Computer literacy and data entry competency including MS Office and demonstrated capacity to learn new technologies including clinical software packages.
* Current driver licence and willingness to travel as part of the role.
