An exciting opportunity exists for a passionate Physiotherapist or Exercise Physiologist to step into a newly established Falls Navigator role within Greater Newcastle Community Services. This is more than a clinical role – it is a unique opportunity to shape how falls prevention services are delivered while directly improving the health and independence of older adults in our community.
About the Role
You'll join a warm and collaborative multidisciplinary team of allied health and nursing professionals led by an experienced Service Manager. The Service delivers cardiopulmonary rehabilitation programmes across Belmont, Toronto, Raymond Terrace, Nelson Bay, Newcastle – the Newcastle Health Centre serves as the home base. The team has a long and successful history delivering evidence‐based programmes including Stepping On, Otago Falls Prevention, Healthy and Active Online Services. With a blend of face‐to‐face and virtual care delivery, there is flexibility and creativity in how services are provided.
As a Physiotherapist or Exercise Physiologist in this new Falls Navigator position, you will have significant scope to influence and shape how the service grows. Your focus will be supporting clients who are at high risk of falls through comprehensive assessment and personalised treatment planning. This role is uniquely positioned to navigate and link clients to the most appropriate services, ensuring care is patient‐centred and clinically appropriate.
* Provide comprehensive falls risk assessments
* Develop individualised, evidence‐based treatment plans
* Deliver care through individual sessions, small group programmes, home visits and virtual healthcare
* Establish and strengthen referral pathways across community services
* Liaise with multidisciplinary teams to coordinate optimal care
* Use project planning skills to support the development and implementation of falls programmes
* Work directly with services better suited to meet specific client needs
About You
We are seeking an engaging and enthusiastic clinician who is genuinely passionate about healthy ageing and falls prevention. Non‐negotiable:
* Professional registration as a Physiotherapist or Exercise Physiologist
* Strong clinical assessment and treatment planning skills
* Commitment to evidence‐based practice
* Excellent communication and interpersonal skills
* Ability to build effective referral pathways and professional networks
* Strong organisational and prioritisation capability
Highly regarded experience:
* Falls prevention programmes
* Cardiopulmonary rehabilitation
* Delivering group‐based or virtual interventions
* Service development and project planning

You will be well supported from day one through a thorough orientation and warm welcome to the team. Regular clinical supervision and support from the Service Manager and Head of Discipline (Physiotherapy) will be provided.
