Key Responsibilities:
* Deliver evidence-based, person-centred, and recovery-oriented care coordination to clients.
* Undertake comprehensive mental health assessments, triaging, and completing risk assessments.
* Develop and implement collaborative care and recovery plans with clients and carers.
* Provide clinical case management, crisis intervention, and therapeutic interventions as required.
* Offer clinical guidance, mentoring, and professional supervision to team members delivering coordinated care.
* Evaluate care needs and develop case plans that address these needs.
