About the Service
The Transitional Nurse Practitioner – Hospital in the Home (HiTH) role provides person-centred care at an advanced nursing practice level within the Hospital in the Home service. Working toward Nurse Practitioner endorsement, the position applies advanced clinical knowledge, assessment, and decision-making skills to support diagnostic and therapeutic interventions while delivering high-quality, patient-focused care in the community.

This role contributes to a district-wide multidisciplinary model of care and works in partnership with medical officers and the broader healthcare team to deliver holistic, integrated services in line with NSW Health Hospital in the Home Policy Directive (PD2025_004).

The position is employed by Southern NSW Local Health District and provides care across the district in a range of settings including patients’ homes, residential aged care facilities, community environments, and the Hospital in the Home (HiTH) unit.

This position is remunerated at the Clinical Nurse Consultant Grade 2 level while the successful applicant progresses through the transitional Nurse Practitioner pathway.

What you'll be doing
As a Transitional Nurse Practitioner, you will provide person-centred care at an advanced nursing practice level while working toward full Nurse Practitioner endorsement. The role involves advanced clinical assessment, care planning, and coordination of patient care, contributing to diagnostic and treatment decision-making within the multidisciplinary team.

You will utilise a high level of clinical knowledge, critical thinking, and professional autonomy to deliver safe, effective, and people-focused care across community and Hospital in the Home settings, supporting improved patient outcomes and reducing avoidable hospital admissions.


Selection Criteria

* Registered Nurse with the Australian Health Practitioner Regulation Agency (AHPRA) working towards Nurse Practitioner endorsement with 5 years post graduate experience and at least 3 years in the relevant field.
* Demonstrated ability to work independently and in the absence of a Medical Practitioner to provide quality care together with the capacity to work collaboratively as part of a multidisciplinary team.
* Demonstrated exceptional clinical communication through the use of information technology, written and verbal means and sound interpersonal skills with the capacity to interact with others in a personable and professional manner.
* Demonstrated comprehensive knowledge in the application of evidence based practice in implementing individualised person-centred care, diagnostic and treatment interventions at an advanced level with a commitment to quality improvement.
* Evidence of participation in own professional development, a commitment to maintaining competence and enhancement of skills, willingness to apply and share professional expertise as a resource person and mentor others within the field of practice.
