Overview
You will be the sole chronic disease specialist supplying ward services to Fitzroy Crossing hospital. You will perform ward rounds and patient discussions with the handover nurse and the ED SMOs. The outpatient ward manages patients with complex chronic conditions (diabetes, chronic kidney disease, ischemic heart disease, rheumatic heart disease, etc.). Previous cultural awareness experience is essential. There is 24/7 support but you will not be required afterhours or over weekends. Travel is via Broome then by air or car to Fitzroy Crossing. All travel provisions including a 4x4 car are provided; you must be able to cover a 2-week block if travelling from outside WA.
Responsibilities
* Provide ward-round assessments and patient discussions with the handover nurse and ED SMOs in Fitzroy Crossing hospital.
* Manage care for patients with complex chronic conditions in the outpatient ward (e.g., diabetes, CKD, ischemic heart disease, rheumatic heart disease).
Qualifications
* FRACGP or FACRRM, Specialist AHPRA registration.
* Previous chronic disease and/or AMS experience.
* WACHS credentialing.
* Previous cultural awareness experience is essential.
