Matthew Flinders Care Services is located in the beautiful Port Lincoln on the lower west coast of Eyre Peninsula of South Australia, known as the seafood capital of Australia. Port Lincoln is a thriving rural community surrounded by beautiful coastline. Local attractions include beautiful beaches and national parks offering an array of outdoor activity, ocean experiences and local seafood dining experiences.
About the role
We are seeking to employ an enthusiastic Enrolled Nurse and Registered Nurse to join our Matthew Flinders Care Services team as a Care Partner and Clinical Care Partner.
Care Partners and Clinical Care Partners are responsible for delivering care management activities that enable older people under the new Support at Home program to live safely and independently at home, providing ongoing planning, monitoring, service coordination, education and direct nursing tasks —with a strong focus on wellness, reablement, and culturally safe care.
The Care Partner works closely with Clinical Care Partner when a client's needs require advanced clinical input or complex care planning.
The Clinical Care Partner ensures that services address both clinical and non-clinical needs. The role combines advanced clinical judgment with care coordination, education, and participant advocacy, embedding wellness, reablement, and cultural safety into all aspects of care.
Both roles are ideally Monday to Friday 8:30 - 4:30pm, offering flexible working hours for the right applicant.

Skills and experience required to be successful in this role
Essential:
* Current registration with AHPRA
* Demonstrated experience in clinical assessment, care planning, and case management in aged care, community, or health settings.
* Solid understanding of the Support at Home Program, Aged Care Strengthened Standards, and Aged Care Statement of Rights
* Competence in interpreting clinical guidance and escalating appropriately
* Strong communication, empathy and cultural competence
* Skilled in using care management or CRM systems
* Current and unrestricted South Australian Driver's Licence.
* The right to work in Australia
* Have a current NDIS Worker Screening or be willing to obtain.
* Pass a pre-employment medical.
* Provide evidence of current Influenza vaccination.
Desirable:
* Experience working with people with dementia, complex ageing-related conditions, or palliative needs
* Training in trauma-informed care, restorative practice, or advanced care planning.
* Familiarity with My Aged Care systems and Support at Home Program requirements.
