As a Community Physiotherapist with us, you'll provide physiotherapy services to customers involved in short-term restorative care, home care packages, and other community programs. You'll work closely with our customers in their own homes to assess their needs and monitor, review and verify the quality and effectiveness of services provided on a regular basis.
Your responsibilities will include maintaining accurate customer records with supporting documentation of care plans, progress notes, customer reviews, outcome measures, incidents, and consent. This is a mobile role, allowing for greater flexibility and autonomy. Administrative tasks can be completed either at home or at a co-working space.
Key Responsibilities:
* Provide physiotherapy services to customers involved in short-term restorative care, home care packages, and other community programs.
* Maintain accurate customer records with supporting documentation of care plans, progress notes, customer reviews, outcome measures, incidents, and consent.
* Meet with customers in their own homes to assess their needs and monitor, review and verify the quality and effectiveness of services provided on a regular basis.
* Develop and implement exercise prescription, rehabilitation, hydrotherapy, strength, mobility, and balance programs.
Requirements:
* Tertiary qualification in physiotherapy and registration with AHPRA.
* Significant practical experience or extensive knowledge in providing physiotherapy services in a rehabilitation and/or aged care setting.
* Effective communication and interpersonal skills.
* Strong organisation and time management skills.
* Demonstrated ability to work independently and as part of an inter-disciplinary team.
* A great attitude, willing to learn new things, and most importantly an interest in listening and understanding what the customer wants.
* Current First Aid and CPR certificate.
* A valid Driver's Licence and willingness to use your own vehicle for work purposes, ensuring it is appropriately insured.
