In this role, you will support participants to live safely and independently while working towards their individual goals. You will play an important role in providing consistent, person-centred support while helping participants remain engaged in their daily routines, home life and community activities.
Responsibilities include:
* Supporting participants with daily living tasks and personal care, including showering, dressing and toileting
* Providing mobility assistance and safe transfers
* Assisting with daily routines, household tasks and meal preparation
* Supporting community access, appointments and social activities
* Following care plans, behaviour support strategies and clinical recommendations
* Working collaboratively with families, allied health professionals and clinical teams
* Completing clear and accurate documentation of supports provided
About You
To be successful in this role, you will have:
* A valid P2 or Open driver's licence and reliable vehicle
* NDIS Worker Screening Check and Working with Children Check
* Experience supporting participants with physical disabilities and personal care
* Experience assisting with mobility support and safe transfers
* Strong communication and documentation skills
* Professionalism, reliability and the ability to build strong rapport with participants
* Availability for a variety of shifts, including mornings, afternoons and overnight shifts
