Key Responsibilities:
* Chronic Disease Management, including Care Plan coordination
* Identifying and participating in Quality Improvement (QI) initiatives
* Contributing to patient education and health literacy
* Administering Immunisations and Vaccinations
* Performing ECGs and Spirometry
* Current registration with AHPRA as a Registered Nurse
* Previous experience in general practice min 2 years
* Immunisation and current CPR certification
* Excellent communication and time management skills
* Warm, professional, and proactive attitude
* Commitment to continuous professional development
* Familiarity with Best Practice software
* Understanding of Medicare item numbers and the Australian healthcare system
The Ideal Candidate:
* Experience in a Practice Nurse role or similar setting
* Strong interpersonal and teamwork abilities
* Confidence using IT systems and clinical software
* Ability to multitask and manage time effectively
* A caring, patient-focused approach
