Registered Nurse (Clinical Care Partner)

About ECHO: ECHO Community Services is a registered charity providing care to the elderly to help them live in their own home with dignity while still maintaining their independence. We provide in-home services 7 days per week for Home Care Package and 5 days per week for CHSP. Our goal is to provide services at the same time, on the same day and with the same team of Support Workers, which means you know the people who are coming and the time they will arrive. Having the same support workers providing services helps maintain the person's dignity and well-being.
Description: The role: The Registered Nurse / clinical care partner is responsible for supporting complex Home Care Package clients, and the delivery of high-quality clinical care services. Your responsibilities may include:
* Building and maintaining strong relationships with clients, carers, and families.
* Developing, implementing, and reviewing individualised care plans to meet client needs.
* Conducting assessments to identify client requirements and ensuring compliance with relevant standards.
* Deliver in-home clinical services, such as medication management, wound care, chronic disease management, and palliative care.
* Acting as a liaison between referral sources, families, and the care team to ensure seamless communication and care delivery.
* Ensuring compliance with all statutory and organisational policies.
* Supporting the care team by providing mentorship and encouraging professional growth.
* Supervise and guide non-clinical team members to maintain high standards of care.
* Participate in the on-call roster when required.
Hours: Monday to Friday, 8:00 am to 4:00 pm. You will also be required to participate in a rotating on-call after hours, weekday and weekend roster.
Essential
* Qualified Registered Nurse with current AHPRA registration.
* Strong organizational skills and a proactive approach to problem-solving.
* Excellent written and verbal communication, with a talent for building relationships and achieving positive outcomes.
* Minimum 2 year's experience in a care management/coordination role within aged care or community care.
Desirable
* Minimum 12 months of experience in a similar role is desirable but not essential.
* Strong interpersonal and communication skills.
* Above average time management and organisational skills.
* Knowledge of aged care and disability sectors.
* Ability to work autonomously and collaboratively within a team.
* Proficiency in care management software and computer systems.
Other
* Current First Aid & CPR certificates.
* National Police Check and valid WA Driver's Licence with access to a reliable vehicle.
