As a Registered Nurse, you will provide exceptional care to clients by ensuring efficient and effective provision of quality nursing care. This role requires direct coordination and implementation of care with appropriate outcomes.
Clinical Responsibilities:
* Triage and client assessment are critical components of this role, along with care planning and clinical data management.
* Effective communication and collaboration are necessary for liaising with Allied Health, Community Care Services, and specialist practices.
* Patient advocacy is essential in facilitating communication between patients and healthcare providers.
Health Screening & Assessments:
* Older person's health assessment is a key aspect of this role, as well as Healthy Kids Check, 45-year-old health check, Aboriginal and Torres Strait Islander health checks, and preventative health checks and programs.
Health Promotion and Chronic Disease Management:
* Providing education and health information to patients is crucial in promoting healthy habits and managing chronic diseases such as asthma and diabetes.
Population Health:
* Immunisation and immunisation recalls are vital in protecting public health.
Requirements:
* A Registered Nurse with current registration with AHPRA is required.
* At least 2 years' experience as a Registered Nurse in an Acute, Community Care, and/or General Practice setting is necessary.
