Job Description
We are seeking a Care Coordinator to join our team. As a Care Coordinator, you will play a vital role in building strong relationships with clients and their families, providing support and guidance to our home care workers, and ensuring that all practices are consistent with policy and legislative frameworks.
Key Responsibilities
* Client Assessments: Conduct comprehensive client assessments and develop care plans to support clients in remaining within their homes.
* Team Support: Interact and provide professional health and clinical care advice and direction to a team of home care workers.
* Liaison: Liaise with clients and their families on a regular basis, addressing concerns and service satisfaction.
* Record Keeping: Maintain up-to-date client records and information.
Requirements
* A formal tertiary qualification (e.g., Bachelor of Nursing or equivalent).
* Current AHPRA registration as a Registered Nurse.
* Demonstrated knowledge within the Aged Care sector.
* National criminal history/police check.
* Industry applicable licences (e.g., AHPRA registration, BC/NDIS).
* A valid First Aid and CPR certificate.
* Minimum of 2 COVID vaccinations.
* An open drivers licence and reliable transportation.
