Your new role
Working within a multidisciplinary team, the Intake & Active Hold Practitioner will oversee the referral, entry and engagement of young people into the Navigator Program, including:
* Supporting intake processes, including initial risk and needs assessments
* Managing a case load of active hold cases and assessing suitability to transition to a case management service
* Engaging and building rapport with young people on the active hold list
* Facilitating referrals to appropriate support services and programs
* Liaising and collaborating with partner agencies, funding bodies and schools

The Navigator Program delivers assertive case management support to young people aged 12-17 years, aiming to re-engage disengaged learners in education. The program utilizes a capability approach that understands how multiple characteristics can interact to compound educational disengagement, leading to disconnection, lack of purpose and self-worth, and exacerbating social and economic disadvantage and isolation.

Your skills and experience
You will meet the person specification in the position description, including having a tertiary qualification in Community Services, Social Work, Education or a related discipline.
You will also have
* Previous experience within the community services sector and/or experience working in a school setting
* Demonstrated experience successfully engaging young people through assertive outreach and case management support
* An understanding of the barriers experienced by disengaged learners
* Proven ability to work collaboratively with other services, agencies and professionals to achieve optimal client outcomes
* Excellent communication and organisational skills
