Job Title: Home Care Partner
The Role
We are seeking a skilled and compassionate Home Care Partner to join our team in Manjimup. This is an exciting opportunity for someone who is passionate about enhancing the lives of older people and their family members/carers.
* Assess support needs, develop and implement individual support plans for Support at Home participants;
* Consult with Clinical Care Coordinators on clinical matters;
* Undertake regular home visits, liaising with participants and family members/carers regarding service delivery;
* Provide regular reports to the multi-disciplinary team, maintaining active communications;
* Identify and manage risks to participant safety;
* Review participant support needs twice yearly;
* Maintain compliance with program standards and reporting;
* Provide guidance and assistance to Support Workers;
* Sources equipment and materials for participants, generating purchase orders and processing invoices as required.
The Requirements
To succeed in this role, you will need:
* Certificate IV in Aged Care or higher, or a relevant qualification;
* Demonstrated experience in coordination and supervision of employees, as well as an ability to train and mentor employees and work in a multi-disciplinary team;
* Demonstrated commitment to the health and wellbeing of older people and people with disabilities and their family members/carers;
* Strong communication skills, able to work with diverse cultures and backgrounds;
* Demonstrated literacy, numeracy, accuracy and attention to detail;
* A high level of initiative, coupled with excellent interpersonal skills;
* An excellent capacity to work within funding constraints.
