Join a Geelong based established NDIS/Allied Health Provider as a Early Intervention Key Worker to deliver quality person and family centred NDIS support services that empower participants to achieve their life goals.

**Your day to day duties will include**
- Provision of therapy in a family-centred approach using a trans-disciplinary model
- Develop, set and review a Family Services Plan for each Participant annually
- Draw on the expertise, knowledge and skills from within the team and deliver trans-disciplinary expertise within the Participant home environment, education environment or another place where the Participant spends time
- Draw on more specialised expertise, liaise in greater detail with the relevant professionals
- Compilation of reports and letters for NDIS Participants, other health professionals and NDIS reporting requirements
**As the successful Early Intervention you will fit the following criteria's**:
- Excellent clinical skills
- Effective time management skills
- The ability and initiative to work autonomously while being part of a clinical team
- Evidence of clinical supervision and ongoing professional development
- Strong knowledge and understanding of the National Disability Insurance Scheme (NDIS) and disability industry
- Valid Australian Drivers License
- Cleared Victorian Working with Children Check (WWCC) or current VIT registration and cleared National NDIS Worker Screen Check
