About The Role
This isn't your average aged care physio role. You'll support clients to build strength, mobility and confidence so they can live their most independent, joyful lives across aged care, home care and disability settings. No two days look the same, and you'll have the freedom to be creative, backed by a supportive team that keeps everything running smoothly.
Responsibilities
* Provide assessment and care planning including core clinical tasks such as assessments, reviews, referrals, and meetings with MDT and staff education.
* Deliver manual handling training to challenge participants and demonstrate knowledge.
* Lead wellness and reablement programs aligned with individual goals and needs, identifying potential participants and enhancing uptake.
* Conduct home visits and community physiotherapy to maintain independence and function.

Duties
* Assess, plan and deliver evidence‐based, goal‐oriented therapy.
* Build a vibrant wellness culture that enhances resident outcomes and engagement.
* Deliver wellness and reablement programs within HELF services.
* Prescribe and document exercise programs for residents, working with on‐site Strength & Conditioning team.
* Build strong client relationships.
* Delivery of therapy through 1:1 and group programs.
* Document clearly and efficiently.
* Collaborate with S&C, clinical and multidisciplinary teams.
* Report and be accountable to HELF service KPIs.
Skills & Experience
* AHPRA registration
* National Police Check
* Driver's licence and reliable car
* NDIS Workers Check and orientation
* COVID‐19 & flu vaccination
* Full working rights
* Aged care experience – desirable
* Private practice experience – desirable
* Home care experience – desirable
