What You’ll Be Doing
Step into a role where your expertise transforms lives across northern NSW.
As a Diabetes Educator & Cardiac Rehabilitation Clinical Nurse Specialist Grade 2 (CNS2), you’ll be a leader in delivering specialised, person-centred care for people living with diabetes and those recovering from cardiac events.
Key responsibilities include:
- Providing individualised, case-managed care for patients across community and acute settings
- Delivering outreach services supporting Warialda, Bingara, Mungindi, Boggabilla, and Toomelah
- Collaborating as part of the Medical Outreach Indigenous Chronic Disease (MOICD) Program, alongside a Dietitian and Aboriginal Health Worker
- Acting as a clinical leader, driving quality care and supporting colleagues within your specialty area

Who You Are
We’re looking for a motivated, autonomous clinician who thrives in rural health and is committed to patient-centred care.
You’ll bring:
- Postgraduate qualifications in Diabetes Management and/or Chronic Care (or commitment to complete within 12 months).
- At least 3 years’ postgraduate experience (consideration will be given to those with less, subject to qualifications and progression pathway).
- Strong communication and collaboration skills with a commitment to patient-centred care.
- A genuine passion for improving the health of rural and Aboriginal communities.
