We have a unique opportunity for a highly skilled and motivated Clinical Hospital Pharmacist to join our dedicated pharmacy team at Epworth Eastern Hospital, Box Hill VIC. This is a permanent full-time position that offers variety, purpose, and professional growth in a dynamic and supportive environment.
As a Clinical Hospital Pharmacist, you will rotate between inpatient wards and the dispensary, delivering high-quality clinical pharmacy services and supporting medication management across a wide range of specialties, including cardiology, intensive care, oncology, renal, and surgery.
Your day-to-day work will include clinical reviews, medication history taking, discharge counselling, and collaborating with a multidisciplinary team to optimise patient outcomes. Whether you're an experienced hospital pharmacist or looking to break into hospital pharmacy, we'll support you with structured hands-on training and development through our National Career Pathway.
About the Role:
In this role, you will:
* Evaluate and manage medication therapy plans for patients across various medical specialities
* Collaborate with healthcare teams to improve patient outcomes and ensure safe and effective medication use
* Develop and maintain relationships with other healthcare professionals to enhance patient care
* Participate in quality improvement initiatives to continually improve pharmacy services and patient care
* Contribute to training and educational sessions for pharmacy staff, nursing staff, medical personnel, and allied health professionals
Key Responsibilities:
The successful candidate will be responsible for:
1. Dispensing and distributing medication efficiently and effectively
2. Conducting medication history interviews, performing medication reconciliations, and preparing discharge summaries during transitions of care
3. Offering medication counselling and education to patients
4. Collaborating with multidisciplinary teams to provide advice on patient care
5. Contributing to training and educational sessions for pharmacy staff, nursing staff, medical personnel, and allied health professionals
6. Assisting in overseeing imprest services and ensuring the continuity of medication supply
