Remuneration: $56.68 - $58.52 per hour
Hours Per Week: 20

What You'll Be Doing
In this role you'll be supporting renal patients and families through complex journeys of care, focusing on holistic, psychosocial and end-of-life support in a multidisciplinary setting.
* Providing psychosocial counselling
* Facilitating advance care planning
* Supporting informed decision-making
* Collaborating with health professionals
* Educating teams and mentoring students

Essential Criteria
* License: Social Worker
* Qualification: Degree in Social Work with eligibility for membership of the Australian Association of Social Work (AASW).
Selection Criteria
* Demonstrated high levels of clinical experience possessing extensive specialist knowledge in Renal and/ or Palliative Care Social work including knowledge and experience in advanced care planning, the psychosocial aspects of caring for dying patients and their families, grief counselling and community support for those impacted by chronic illness and increasing decline in health status.
* Demonstrated experience in staff and/or student supervision.
* Experience in developing and implementing policies and procedures for service improvement and skills in providing training and education.
* Well-developed time management skills with excellent written and verbal communication, including report writing skills and high-level computer literacy.
* Demonstrated excellent negotiation, decision making and advocacy skills including the ability to interact constructively and collaboratively with a diverse range of stakeholders, throughout all organisational levels.
* Demonstrated ability to initiate, lead, complete and implement quality improvement initiatives and service evaluation processes and lead ongoing quality improvement activities with other staff.
* Demonstrated understanding of issues associated with working with Culturally and Linguistically Diverse client group.
