DV-RRH Case Manager - DV Rapid Rehousing ProgramUnder the supervision of the Domestic Violence Rapid Rehousing department, the Case Manager provides services to homeless and formerly homeless individuals and families fleeing domestic violence or human trafficking. The role involves using evidence-based practices such as Trauma-Informed Care, Motivational Interviewing, Harm Reduction, and Housing First.The primary responsibilities include developing and implementing goals in the client's Individualized Service Plan (ISP), assisting clients in maintaining housing, and addressing issues like mental health, substance use, income, and interpersonal relationships.
