This is a key clinical leadership role supporting innovative models of care that provide safe, effective alternatives to hospital‑based treatment. Dhelkaya Health's At Home Program provides multidisciplinary clinical care in the home or place of residence, when safe to do so. The program operates 7 days a week and includes: * Hospital in the Home (HITH) * Geriatric Evaluation Management (GEM) at Home * Residential In Reach (RIR) * Rehabilitation in the Home (RITH) The program aims to deliver the right care, in the right place, at the right time, improving patient flow, consumer experience, and health outcomes. The At Home Nurse Coordinator is a Clinical Nurse Consultant role responsible for delivering expert nursing care and coordinating At Home services across all phases of care. You will advocate for clients, support their health goals, and ensure coordinated, high‑quality multidisciplinary care. This role plays a pivotal leadership function, overseeing referral screening, triage, intake, assessment, care planning, review, transition, and discharge processes. You will also have a key role in recognising and responding to acute deterioration and supporting safe transitions to other services when required. Key Responsibilities * Provide expert clinical nursing care to clients in the home or residential setting * Screen referrals, triage, coordinate intake, and manage access to At Home Programs * Coordinate timely admission, assessment, and implementation of individualised care plans * Monitor, review, and coordinate transitions and discharge planning * Recognise and respond to clinical deterioration and escalate care as required * Advocate for client choice and support carers throughout the care journey * Build capability through staff education and promotion of At Home care pathways * Establish and maintain strong collaborative relationships with internal and external stakeholders * Work closely with inpatient, residential care, district nursing, and community outpatient teams * Support value‑based health care principles to enhance client outcomes, experience, and staff wellbeing About you * Bachelor of Nursing with current AHPRA registration * Minimum 3 years full time equivalent working as registered nurse in an acute setting or related setting * Current Victorian drivers' licence Culture and Benefits Dhelkaya Health is located on Dja Dja Wurrung country in regional Victoria.
