About the role
Catholic Homes is seeking experienced Registered Nurses to join our Home Care team on a casual basis, servicing the Geraldton area.
Our Home Care team builds lasting relationships with our clients so that they may stay at home or get out into the community to do the things they enjoy.
The successful applicant will receive ongoing training from management and the senior care team.
Key responsibilities
Follows clients' Care Plans and conducts clinical care requirements/assessments;
Carries out clinical assessments as directed
Follows care plans to clearly direct clinical care requirements and reporting requirements
Recognise and respond in a timely manner to client's changing health care needs including initiating an assessment, review or appropriate referral in consultation with client/family/advocate
Supports the Home Care Team by recommending reassessment of level of care, or where the care needs have exceeded the constraints of the current package
To be successful in this role
Current Registered Nurse AHPRA registration - no restrictions
A roadworthy and registered vehicle and current driver's licence.
Excellent communication and assessment skills
Available to work across a variety of shifts including weekends
National Police Clearance
Aged Care experience desirable
