Your Role
As a Co-Morbidity Case Worker, you will play a key role in delivering specialist mental health and psychosocial support to clients accessing Rosalie House, Freeman House and the New England Alcohol and Other Drug Outreach Program. You will support individuals experiencing complex needs, including co-occurring mental health and alcohol and other drug challenges, helping them navigate services and work towards improved stability and wellbeing.
Your Responsibility
Provide trauma-informed psychosocial support to individuals with co-morbidities
Deliver end-to-end case management across engagement, assessment, treatment planning, implementation, monitoring, review and closure
Conduct intake and comprehensive client assessments
Deliver therapeutic interventions, education and practical support
Work collaboratively with clients' carers, families and significant others, including education, brief interventions and referrals
Participate in individual and team-based case reviews
Maintain accurate and timely client records and case notes
Build and maintain strong working relationships with internal teams and external stakeholders
Contribute to general operational duties as required
Undertake additional responsibilities as directed by the Regional Manager
You Will Need
A relevant qualification such as Certificate IV or Diploma in Alcohol and Other Drugs, Mental Health or Community Services, or substantial experience in the community services sector
Strong interpersonal and communication skills with the ability to work across diverse cultural contexts
Lived experience related to substance use and or mental health
Demonstrated experience managing complex caseloads and supporting clients with multiple needs, including mental health, homelessness or justice system involvement
Ability to work within the Society's AOD Model of Care and NSW Health AOD Clinical Care Standards
Sound knowledge of community services and referral pathways
Ability to work both independently and as part of a multidisciplinary team
Strong organisational, time management and documentation skills
Basic computer literacy and experience with electronic data systems
