About the Role
This position involves delivering personalized wellness and lifestyle services that cater to the unique needs and aspirations of our residents.
You will be responsible for designing and implementing engaging programs that promote social interaction, physical activity, and mental stimulation, ensuring a high quality of life for our residents.
In this role, you will work collaboratively with colleagues to develop and implement Recovery Pathways for residents at risk of decline and frailty, utilizing best practice tools and evidence-based approaches.
* Support and enable residents to maximize their independence and improve their health, wellness, and quality of life
* Utilize best practice and evidence-based tools to plan and deliver high-quality lifestyle and exercise services, emphasizing person-centered care and healthy aging through goal setting, emotional support, and health literacy
* Enable participation in community and facility activities relevant to each resident
* Contribute to a multi-disciplinary Early Intervention team to identify and develop Recovery Pathways for residents at risk of decline and frailty
* Audit residential Lifestyle programs to ensure they operate in accordance with established procedures
* Coordinate staff, volunteers, and students to deliver meaningful lifestyle activities and projects
About You
We are seeking individuals who share our commitment to enhancing the lives of our residents and clients. The ideal candidate will possess:
* A minimum Diploma-level qualification in health promotion, social work, dementia care, health coaching, or a related field
* Demonstrated experience in auditing and reporting, documenting person-centered assessments, planning, developing, implementing, and evaluating wellness and lifestyle programs for older adults
* Sound best practice skills for early identification of social withdrawal and decline, as well as case management to improve resident outcomes
* A good understanding of requirements to support residents living with dementia, including those displaying challenging behaviors
* Strong customer service focus, with experience in undertaking regular consultation with clients and their families
* Exceptional communication, organizational, and relationship-building skills
* Sound problem-solving skills, particularly when assessing balancing supporting residents' desire for independence and self-determination against potential risks and duty of care
* A current Driver's License
* Proficient computer skills and the ability to use Google Suite and in-house databases
