What You'll Be Doing
The Senior Clinician Community Health will be responsible for providing support to patients with late stage chronic and complex health conditions including:
* Care planning and advance care planning
* Palliative care needs assessments and collaborative treatment care plans
* Referrals to relevant internal and external health and community support providers
* Education/support to other clinicians in supporting complex patient needs
Selection Criteria
* Current Registration as an Occupational Therapist with AHPRA or eligibility for accreditation as an Accredited Exercise Physiologist with ESSA
* Extensive knowledge and experience working with consumers with chronic and complex illnesses, including advance care planning discussions
* Experience in care planning and case management of patients in the community with chronic and complex illnesses
* Understanding of specialist palliative care, a palliative approach and community health services
* High level interpersonal and communication skills, including written, verbal and computer skills, and ability to work in a multidisciplinary team
* Clinical leadership capabilities with the ability to provide consultancy, education and support to individuals, families, allied health and nursing staff, and other health professionals
* Current unrestricted drivers' licence and ability to drive/travel as required (may involve long distances and overnight stays)

Occupational Assessment, Screening and Vaccination
