This position plays a crucial role in delivering high-quality care to individuals at risk of readmission.
* Provide specialized physiotherapy services to clients with complex needs
* Contribute to personalized client care plans in collaboration with healthcare teams
* Foster effective working relationships with multidisciplinary teams to enhance patient outcomes
* Qualifications in Physiotherapy or equivalent experience
* Registration with relevant professional bodies
* Proven experience in acute or sub-acute settings, preferably with a focus on rehabilitation and recovery
