About us

Carepro Disability Services is a values-driven NDIS service provider with offices based in Perth and Melbourne. We're a small, vibrant and supportive team passionate about helping people live meaningful and fulfilling lives. Carepro is entering a deliberate growth phase, and we are looking for the right Client Service Coordinator to join us and build alongside us — someone who can take ownership, hit the ground running, and help us shape the future of our service delivery.

About you

You are organised, confident, and deeply committed to delivering high-quality, participant-focused support. You thrive on building something meaningful from the ground up and enjoy the challenge of shaping relationships, systems, processes, and teams that make a real difference. You understand the balance between compliance, participant outcomes and sustainable service delivery.

You'll Bring

* Minimum 2 years' experience in Disability or NDIS Service Coordination
* Strong working knowledge of the NDIS framework, Practice Standards and Code of Conduct
* Experience supporting participants with moderate to complex needs
* Demonstrated ability to build and grow a caseload
* Confidence working independently with strong organisational discipline
* Existing referral networks (highly regarded)
* Sound understanding of restrictive practices and reporting requirements
* Strong report writing and documentation skills
* The ability to build rapport quickly while maintaining professional boundaries
* A proactive, resourceful and solutions-focused mindset
* NDIS Worker Screening Clearance
* Working With Children Check (WA)
* Unrestricted Australian work rights
* Current driver's licence and reliable vehicle

Key Responsibilities of the role

* Lead end-to-end participant engagement, intake and onboarding
* Interpret NDIS plans and funding to maximise supports and outcomes
* Design and implement individualised supports aligned to participant goals
* Proactively build and grow your own participant caseload
* Develop and maintain referral networks
* Match participants with the right support workers and oversee service delivery
* Support participants through plan reviews and changing needs
* Ensure services reflect genuine choice, control and dignity
* Understand and implement Behaviour Support Plans in line with the Positive Behaviour Support Framework
* Roster in accordance with the SCHADS Award
* Maintain compliant documentation including Service Agreements, risk assessments, incident and complaints management
* Drive continuous improvement in line with NDIS Practice Standards

If you're looking for a role where you can lead, grow and genuinely influence participant outcomes — we'd love to hear from you!
