About the Program

The Homes First Program is an exciting, multi‐year pilot initiative designed to support single adults, families, and young people who are experiencing homelessness or rough sleeping. Our goal is simple yet powerful: secure safe, long‐term housing first, then wrap around the flexible, culturally safe and trauma‐informed supports needed for individuals and families to thrive.

Guided by the internationally recognised Housing First principles, our work reflects the belief that:

* Everyone has a right to a home
* Housing and support are separate
* Support is flexible and available as long as needed
* Individuals have choice and self‐determination
* Engagement is active, without coercion
* Practice is recovery‐oriented
* Social and community inclusion matters
* A harm‐reduction approach underpins all support

The program spans three years of case management, with an additional year of intermittent support to ensure ongoing stability. The outcomes we strive for include long‐term housing, improved mental and physical health, and strengthened connection to community.

You will join a passionate multidisciplinary team that includes:

* Team Manager
* Homes First Case Managers
* Aboriginal Case Managers
* Dual Diagnosis Clinicians
* Peer Support Workers

Together, we work toward a future where homelessness is rare, brief, and non‐recurring.

The Role – Homes First Case Manager

We are seeking a compassionate, skilled, and values‐driven Homes First Case Manager to support individuals and families to secure and maintain safe, sustainable housing. This role is part time (0.8 FTE) 60.80 hours per fortnight on a fixed term basis till June 2027.

In this role, you will:

* Provide flexible, trauma‐informed, culturally safe case management
* Complete comprehensive needs assessments and develop client‐led support plans
* Offer practical support to individuals and families, including accompanying children
* Connect clients with our multidisciplinary team and external service providers
* Support clients to strengthen daily living skills where there are service gaps
* Promote self‐determination, especially for Aboriginal clients, including connection to Country and community
* Champion a recovery‐focused, harm‐reduction approach
* Work collaboratively to prevent future episodes of homelessness

* Experience supporting vulnerable individuals or families
* The ability to build trust, empower clients, and work respectfully within diverse cultural contexts
