The Specialised Transitional and Restorative Support Service (VITA STARS) is a 40-bed ward located in the VITA building at the Repat Health Precinct. The service offers guidance and support with clients' goal setting and a range of therapies (such as physiotherapy and occupational therapy) by using a low intensity approach to help achieve the clients' goals, and assistance with personal care and nursing support as required. VITA STARS offers clients a low-acuity restorative program of care in a home-like environment with a focus on function to enable safe transition back into the community after an acute episode of care.
As the Admission and Discharge Nurse, you will be responsible for the proficient, person centred, clinical nursing care and individual case management to patients in the Specialised Transitional and Restorative Support Service. You will support patient flow and promote the continuity and consistency of care. You will assist the Nurse Unit Manager in undertaking service management responsibilities,, recruitment, staffing, leave management, rostering, work allocation and attendance management; financial and supplies planning and monitoring.
To be successful in this role, you will have:
3. at least 3 years, full time equivalent, post registration experience
4. experience and competence in rehabilitation and geriatric nursing practice in accordance with the relevant standards
5. effective communication, problem solving, conflict resolution and negotiation skills
6. Experience in the leadership and direction of student nurses, enrolled nurses and less experienced registered nurses
