Provide high level Social Work services within Mental Health, Hospital in the Home (HITH) Program. The Senior Social Worker will undertake complex social work clinical services for psychosocial assessment, treatment intervention, care planning and discharge planning of consumers experiencing and affected by serious mental health issues and challenges across the Gold Coast to promote, maintain and improve their mental health and recovery and quality of life.
The Role
An opportunity has become available for a Social Worker - Senior (Mental Health - Hospital in The Home) to join Gold Coast Health (GCH) where the focus of our 13 000-strong team is on providing world-class health care to people living in the Gold Coast, and northern New South Wales region of Australia.
You will
* Have at minimum four years provision of high level clinical experience, knowledge and skills, in delivering an efficient and responsive Mental Health Social Work service in accordance with professional and ethical standards
* Identifying and applying differential assessment methods for complex psychosocial needs of consumers, families and carers and partnering inclusively to promote consumers human rights
* Apply high level advocacy skills with proficiency in navigating complex healthcare and social systems to support consumer needs
* Undertake high level therapeutic interventions using evidenced based social work and mental health theories, therapies and frameworks for practice for care planning and discharge planning
About you
To be successful in this position you will require:
* A tertiary degree in Social Work
* Eligible for membership to the Australian Association of Social Workers (AASW)
* Driver licence
* Evidence of vaccination, or be able to be vaccinated against and remain vaccinated against - Measles, Mumps, Rubella, Varicella, Pertussis and Hepatitis B
* Tuberculosis screening, to identify if the tuberculosis vaccination may be required
* Declaration of serious discipline history must be disclosed
* Criminal History Screening will be required for new employees (GCH will cover cost)

* Permanent part time (60.8 hours p.f)
* Base salary ranging from $64.64 - $69.55 per hour

Mental health and Specialist Services (MHSS) provide a range of hospital and community based services that seeks to promote and integrated specialized system of care. The Mental health Hospital in the Home (HITH) Program is a service that offers contemporary recovery orientated care and forms part of the Acute in Patient service model. HITH provides an alternative to an inpatient hospital admission, which allows MHSS to offer a more person-centred option to voluntary admission for mental health consumers. HITH acts as a step between inpatient hospital admission and community care where the intensity and type of care provided in an admitted service is offered but allows for greater flexibility to meet the patient's preferences and facilitates maintenance of supports and links within the community.
